Creative Health Care Management (CHCM) is a private U.S. corporation, which provides consultation and training in the Health Care sector. CHCM is based in Minnetonka, Minnesota.

Consulting Practice
CHCM was originally established to support the implementation of Primary Nursing, by translating the relevant transformational change principles from the original implementation into consultation techniques.
The next subsequent step for the organization was the development of a coordinated curriculum to prepare staff nurses to become managers. Originally a five-day program, it was shortened to three days and continues through to today. It is licensed to organizations around the world, who do their own 'train-the-trainer' trainings and then are able to provide the content themselves.
CHCM supports the best practices of the profession and serves as a hub for innovative practice. Integral to those innovations are partnerships. "Nursing.. recognizes that working collaboratively with physicians and other health care providers is essential. We are redefining our relationships with physicians as a partnership, understanding that it is our right and responsibility to manage patient care within the framework of that partnership and that of an ever-changing health care system. But our primary relationship has been and must always be with the patient."

Nursing literature
The Practice of Primary Nursing was first published in 1980 by Blackwell Scientific. It was reprinted multiple times, then in 2002 CHCM printed a new edition which the American Journal of Nursing honored as a 'Book of the Year' for 2002.
In 2004 CHCM published 'I2E2: Leading Lasting Change,' by Jayne Felgen, which also won the honor of being named a 'Book of the Year' for 2004 by the American Journal of Nursing.
Also in 2004, CHCM released 'Relationship-Based Care: A Model for Transforming Practice' which took the original concepts of Primary Nursing, expanded them to all functions and relationships within the hospital setting, and centered the model more fully within all the accumulated practices and beliefs of its professionals over the years. The RBC book has been translated into German and other languages
CHCM published 'See Me As a Person: Creating Therapeutic Relationships with Patients and Their Families' by Mary Koloroutis, RN and Michael Trout in 2012.
In 2016 CHCM published 'Primary Nursing: Person-Centered Care Delivery System Design' by Susan Wessel and Marie Manthey, which was named a 'Book of the Year' by the American Journal of Nursing.
CHCM published 'Advancing Relationship-Based Culture' by Mary Koloroutis, RN and David Abelson, MD. Advancing Relationship-Based Cultures explores the confluence of relational and clinical competence that advances relationship-based healing cultures.
Leadership
CHCM is committed to a shared leadership model, operating with a board, two executive leaders, a financial officer, and a CEO. Self-directed teams made up of members within the organization advance innovation in products and services, operational effectiveness research and internal development.
